1)INTERPRETATION OF PERICARDIAL FLUID ANALYSIS



Use of light’s criteria in interpreting pericardial fluid is irrational as going by the study being provided here where in 30 patients with no pericardial disease undergoing CABG and Valve replacement surgery were analysed.

The mean fluid LDH was 2.4times higher than sr LDH

And mean fluid protein 0.6 times lower than Sr protein.

The avg leucocyte count was found to be more than 1400…

In the application of light’s criteria these might have been labelled as exudative causes.


Hence light’s criteria has poor diagnostic value for pericardial fluid

2) ACCURACY OF ACTH STIMULATION IN DIAGNOSING ADRENAL INSUFFICIENCY


102 patients included in the study


Out of which 66 patients showed a normal response to ACTH stimulation, with peak cortisol levels greater than 500 nmol/L (18 µg/dL). We considered them as responders (Group A). 


The remaining 36 patients showed a peak cortisol level below 500 nmol/L; hence, they were considered to be non-responders (Group B). 


After the execution of ACTH stimulating test, every patient was clinically followed. Four patients in the Group B were diagnosed for AI (true positive): 


2 were Primary Adrenal insufficiency 

2 were Secondary AI.

3)MEGALOBLASTIC ANAEMIA IN OBESE

1252 patients with morbid obesity that randomly selected from all patients referred to Clinic of obesity at Rasoul-e-Akram Hospital in 2014.The overall prevalence of iron deficiency anemia was 9.8%. The prevalence of iron deficiency anemia was independent to patients' age and also to body mass index. The prevalence of vitamin B12 deficiency was totally 20.9%. 

Although this is in patients with BMI of 40 and above

4) ATRA THERAPY IN AML

Twenty nine patients with newly diagnosed AML-M3 (acute promyelocytic leukemia) based on morphology according to FAB (French-American-British) association enrolled between December2003 to June 2006...

ATRA therapy in dose of 25 mg/m 2 /day in two divided doses for 4 - 6 w eeks along with anthracyclin - based chemotherapy...

Twenty one patients ( 73%) achieved complete remission, with induction related mortal i ty of 27% (8 patients of 29 ) . Three died because of ATRA syndrome, three due to sepsis and the other two because of intracranial hemorrhage

5)Reliability of MRI as first line investigation for Meniscal tear?


A retrospective study analysing 2998 pts who had undergone arthroscopy for medial or lateral meniscal tear with or without ACL rupture on MRI ,showed that ,out of 543 pts who were eventually included in the study( the remainder excluded based on exclusion criteria ), 

261 had medial meniscus tear(MMT)  and 271 had lateral meniscus tear(LMT)

On the contrary MRI was suggestive of MMT in 302 pts and LMT for 258 pts

The type and location of tear on arthroscopy was based on ISAKOS classification- International Society of Arthroscopy, Knee Surgery, and Orthopedic Sports Medicine.
